Overview

The Antimicrobial Stewardship Pharmacist is responsible for providing pharmaceutical care by working collaboratively with medical staff, pharmacy staff, and additional departments to provide safe, cost effective use of antimicrobial agents to promote appropriate treatment for acute and chronic infectious diseases. The pharmacist will assist with antimicrobial stewardship services and providing care to inpatients.

 

Schedule 

Full Time, Day Shift

7:00am-3:30pm

$30,000 Sign-on Bonus 

Qualifications

Education:

Graduate of an ACPE-accredited School of Pharmacy required

 

Licensure/Certification/Registry:

Current license to practice Pharmacy in the state of Illinois required

Advanced Pharmacy Residency Training (PGY-1) preferred

Infectious Disease Certificate Program required within 2 years

 

Experience:

Five years hospital pharmacy experience required, with at least three years at Springfield Memorial Hospital preferred.

Must demonstrate excellent collaboration and experience within multidisciplinary teams.

Must demonstrate excellent leadership and contribution to antimicrobial stewardship.

Must demonstrate all values of the organization as demonstrated through work history and documented on performance review.

 

Responsibilities

  1. Participates in the planning, design, implementation, and value management of new and existing antimicrobial stewardship initiatives while seeking new opportunities to improve Antimicrobial Stewardship to maintain compliance with TJC and CMS.

 

  1. Will assist in staff education of clinical activities as they relate to antimicrobial stewardship.

 

  1. Actively participates in committees, councils, and task forces in order to promote pharmaceutical care goals and patient safety. This includes, but is not limited to, Pharmacy and Therapeutics committee, Antimicrobial Stewardship Sub-Committee to Pharmacy and Therapeutics, and Infection Prevention and Control.

 

  1. Precepts Clinical Trainees (i.e. pharmacy students, PGY-1 residents, etc)

 

  1. Rounds with ID consult service and serves as the antimicrobial therapy drug expert.

 

  1. Conducts daily prospective audit and feedback of antimicrobials.

 

  1. Is familiar with, understands, upholds, and complies with all policies and procedures of the hospital and the department.

 

  1. Serves as a clinical liaison for antimicrobial stewardship between the pharmacy staff, medical staff, nursing, and other hospital departments.

 

  1. Performs all functions of a practicing pharmacist, as needed. This includes dispensing orders for medications, monitoring patient drug therapy, and providing drug information.

 

  1. Provides training to new staff, as needed.

 

  1. Takes responsibility for personal and professional growth. Provide annual documentation of professional self-development (i.e. relevant continuing education, professional meeting attendance, certification)

 

  1. Performs other duties as assigned
