Overview

The Staff Physical Therapist II is responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines.  Serves as a role model in the delivery of professional services and is a clinical resource for staff and students.  Embodies the Memorial Health System Performance Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values

 

Bachelor’s degree in Physical Therapy or entry level equivalent is required.  Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state.  Current CPR card.

Qualifications

Licensure/Certification/Registry:

  • P.T. license in the State of Illinois required.
  • CPR Certification preferred.
  • If applicable, must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (5 year MVR will be required). Must have availabilty of personal vehicle with proof of insurance. 

 

Experience:

  • Minimum of two years of experience in Home Health or five years general clinical experience required.  

 

Other Knowledge/Skills/Abilities:

  • Consistently meets expectations at the staff level I for patient care and clinical skills.
  • Universal Precautions Category I. 
  • Will accept a MHSvcs car for business purposes only as deemed necessary by geography/mileage. 
  • Achieve and maintain 9-14 points on Rehab Clinical Ladder grid.

Responsibilities

  1. Serve as a role model in the delivery of P.T. treatment.  
  2. Provides high quality patient care in service areas/clinics as assigned.  
  3. Facilitates effective team work and participates in the clinical development and supervision of assistants.  
  4. Pursues self-education and maintains skills attending continuing education programs, department in-services, reviewing literature and research in order to stay abreast of clinical developments in the field.

  3. Complies with Agency standards for productivity in order to ensure efficient service delivery and fiscal accountability.  
  4. Participates in continuous quality improvement (Quality Initiative measures) including efforts to improve patient outcomes. (Oasis)

  6. Is accountable for all equipment in his/her position including but not limited to, laptop computer, FAX machine, and Rehab equipment.  
  7. Evaluates treatment needs of assigned physician referred patients.   
  8. Determines and implements plan to accomplish treatment goals within appropriate time frames.   
  9. Provides appropriate feedback to physician, department, and other team members and maintains required documentation per department policy.   
  10. Contributes to the maintenance of a satisfactory work environment.  
  11. Assists in development of physical therapy programs.  
  12. Performs other related work as required or requested.
