The Manager, Care Management- ACP & Palliative Care (Manager) is responsible for managing the day-to-day operations of the advance care planning and palliative care teams. This include ensuring effective care coordination and collaboration with the interdisciplinary team, analyzing the department’s use of internal and external resources in order to achieve optimal patient outcomes, promoting high-quality, cost-effective patient management, optimizing department resources, training team members, mentoring colleagues, and reviewing colleague performance. The Manager leads the team to work collaboratively with the medical team to meet the care management aspects of a patient’s illness and ensure resources are in place to support patients and caregivers.
Education:
· Master of Nursing degree from an accredited College of Nursing
OR
· Master of Social Work degree from a School of Social Work accredited by the Council on Social Work Education
OR
· A Master of Healthcare Administration, Business Administration, or related degree required.
Licensure/Certification/Registry:
· Current licensure as a Registered Nurse (RN) in the State of Illinois required with Master of Nursing degree
OR
· Current licensure as a licensed social worker (LSW) in the State of Illinois required with Master of Social work degree
Experience:
• Management experience preferred.
• Evidence of experiences, which define competence in personnel management and program development.

• Evidence of leadership, creativity, integrity, and initiative.
• Must possess strong oral and written communication skills, planning skills, problem-solving skills, and change management skills.
• Extremely strong skills in personal diplomacy; demonstrates personal traits of a high level of motivation, team orientation, professionalism and trustworthiness.
• Must possess sound decision-making ability, operate well under pressure and be tenacious in resolving difficult problems.
• Demonstrated ability to deal effectively with staff at all levels of the organization required, including medical staff.
• Support the department director on all key strategic initiatives and daily operational tasks
