Our Psychiatric Counselor provides case coordination services to psychiatric patients of all ages and their families. Key responsibilities include conducting Comprehensive Psychiatric Social Histories, offering individual, group, and family counseling, and managing discharge planning. The counselor also facilitates the multidisciplinary team’s collaboration through the delivery of specialized departmental services.

Education:

Master’s degree in Clinical Psychology, Counseling, or a related field.

Licensure/Certification/Registry:

Preferred: Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
Minimum requirement: Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC).

Experience:

Preferred: One to three years of supervised clinical experience in a psychiatric setting or a related field.

• Conduct psychiatric social services assessments within 3 working days of admission, ensuring all regulatory mandated information is included and documented in the patient's record.
• Provide group, individual, and family counseling to patients of all ages, as requested by the physician or multidisciplinary team, documenting each session and supporting the Utilization Review criteria and patient treatment plan.
• Initiate discharge planning upon admission, coordinating with the physician and interprofessional team. Ensure discharge plans are in place when the patient is medically ready and that all relevant information is shared with the next care provider.
• Maintain knowledge of community resources to support patient care.
• Comply with the Illinois Mental Health Code and Confidentiality Act.
• Take a leadership role in interprofessional treatment team meetings.
• Consult with colleagues for individual treatment planning for complex patient cases.
• Provide evidence of ongoing professional development through continuing education.
• Assist in staff training, new employee orientation, student education, community education, in-house activities, and public relations efforts as needed.
• Follow department and hospital policies and procedures consistently.
• Demonstrate knowledge of care for older adults by performing accurate assessments, providing appropriate treatment, and implementing effective interventions.
• Perform other duties as assigned.
