Overview

Nursing professional ensuring coordination and continuity of care for patients involved in the kidney transplant programs. The transplant coordinator works with physicians, partner clinic personnel (SIU, Springfield Clinic), dialysis units and hospital personnel in a team effort to identify, prepare and educate the potential transplant recipients and living organ donors. Provides nursing care in a clinic setting for transplant patients pre and post-operatively, overseeing their care for the life of the kidney, as well as living organ donor patients within protocols developed by the transplant team under direction of the Medical and Surgical Directors. The nurse will ensure compliance with all regulatory requirements and national standards for transplant programs.

 

Organ Offer On-Call Responsibility: As part of the on-call rotation, that provides 24/7 on-call responsibility for handling organ offers for transplant at SMH.

 

The nurse will report directly to the Manager, Transplant, Dialysis and Therapeutic Apheresis Services and will also take direction from the program’s transplant and nephrology physicians.

Qualifications

Education:

· Degree from an approved Baccalaureate Nursing Education program preferred.

Licensure/Certification/Registry:

· Registered Nurse with current State of Illinois license.

Experience:

· Two years of experience in the care of nephrology patients strongly preferred.

· Two years clinical nursing experience required.

· Meets credit range for Clinical Ladder Levels as follows:

o Transplant Nurse I: Registered Nurse with at least 12-18 months experience as a registered nurse who

achieves a range of 1-12 credits on the Clinical Ladder Behavior Grid.

o Transplant Nurse II: Registered Nurse who achieves a range of 13-24 credits on the Clinical Ladder

Behavior Grid with evidence of behaviors from at least two categories of the Grid.

o Transplant Nurse III: Registered Nurse who achieves a range of 25-35 credits on the Clinical Ladder

Behavior Grid with evidence of behaviors from at least three categories of the Grid.

o Transplant Nurse IV: Registered Nurse who achieves at least 36 credits on the Clinical Ladder Behavior

Grid with evidence of behaviors from at least three categories of the Grid.

· Experience with clinical research preferred.

Other Knowledge/Skills/Abilities:

· Proficient computer skills required.

· On-Call responsibility required. Must possess ability to present key organ offer data to Transplant

Surgeon on call in order to efficiently evaluate organ offers.
