Overview

Provides services designed to deflect unwarranted placement into institutional or long-term setting.  Provides direct service to clients who have exhibited low frequency or minor adaptive behaviors, or who have manageable physical impairment, or who need ongoing rehabilitation programming

Qualifications

Education:

High school diploma/GED.

 

Licensure/Certification/Registry:

DSP preferred but not required.

 

Experience:

A minimum of one year experience working with developmentally disabled individuals. 

 

Other Knowledge/Skills/Abilities:

Must have keyboarding and computer skills as required by position. Must have valid IL Driver’s License, reliable transportation, clean driving record, and proof of automobile insurance. Must be able to pass background check and drug test. Must be able to lift and carry at least twenty-five pounds for storing of commodities, provide CPR/First aid if needed, and lift clients if required.

Responsibilities

  1. Provide support to persons with developmental disabilities, their natural family, guardian or foster family in order to maintain the individual in the least restrictive alternative in a crisis or routine situation.
  1. Promote a living situation which is supportive and strengthening with the goal of helping residents by training/instructing recipients according to Individual Service Plans initiated for/by clients and/or their guardians to move to higher levels of personal responsibility. (Budgeting, maintenance of residence, meal planning and preparation, and daily living skills.)
  1. Attend client staffing and related agency meetings as requested by supervisor.
  1. Provide emergency services when warranted.

  1. Participate in developing intramural and extramural activities with emphasis on community orientation (recreational and socialization resources and opportunities).
  1. Assist in relating information regarding the facility’s expectations of its clients to other community resources.
  1. Provide clients with orientation, including expectations and procedures within the facility.
  1. May assist in planning and preparation of staff meetings with sponsoring agencies, and in developing individualized program goals. Complete all required progress and evaluation forms and attend staff meetings as assigned.
  1. Supervisor client programs as determined in the IHP, making further client assessments based on progress achieved.
  1. Transport clients as needed and requested.
  1. Complete required administrative forms (time off slips, timesheet, expense vouchers).
  1. Provide assessment of clients’ needs required for developing and coordinating a written individual treatment plan.
  1. Oversee recordkeeping for client financial management.

  2. Support importance of clients taking their medications.
  1. Take all necessary steps to ensure that sanitary practices are followed in purchasing, storing, preparation, and serving of meals and maintenance of food service area.
  1. Adhere to MBH/MHS Behavioral Standards
  1. Complete all mandatory education requirements by specified deadlines.
  1. Perform other duties as required by agency needs.
